HOW TO FILL OUT A
REGIONAL LIBRARY CARD APPLICATION FOR YOUR CHILD
PLEASE READ BEFORE FILLING OUT A CARD.
IF THE APPLICATION IS NOT FILLED OUT
CORRECTLY YOUR CHILD WILL NOT BE ISSUED A LIBRARY CARD.
To get your child a library
card a parent must have a card as well. We require an application be filled
out for each family member needing a card. Parents are required to fill
out cards for children under the age of 18 who do not have the required
two forms of identification. Please understand that when you fill out an
application for your child, you are accepting financial responsibility
for the materials that they check out of the library.
Please have you or your
child fill out the following information in the provided area of the application:

Name (written legibly)

Date of birth
-
Mailing address (If you have a PO BOX we must
have a physical address as well)
-
Phone number
-
Social security number (Used for identification
purposes only)

Signature of applicant
Fill out both sides
of the card as the parent’s signature, library card number and the date
is required on the back of the card in the box indicated.
A parent must accompany
their child when returning with the application for library card issue.
We require the parent to show two forms of ID that verify your name and
address. One must have your picture (i.e. - driver’s license, etc.) and
one must have your current address (i.e. - mail with current address),
or both.
There is no charge for
the first regional library card issued. If it is lost a new card can be
issued for the replacement cost of $2.00.
